Online Dues Renewal Form

This form is for renewing members only.
If you are a new member wanting to join the Louisville Federation of Musicians 11-637, you can register here.

Online payments are processed with PayPal. You do not need a PayPal account as PayPal will allow the use of a credit or debit card. PayPal charges the local a small fee for each transaction which is added to your dues. Members may also bring or mail a check or cash to the Union office, which is not subject to the PayPal fees.

If paying by mail, be sure to include:

  • Member full name.
  • Full name(s) of additional members.
  • The number of engagements for which work dues are owed.

Regular members may pay dues in two installments. students may pay dues in four installments. Paying in installments is not available if paying by PayPal.
